Please enter the name of the gift certificate recipient when checking out > When you go to your cart, please click on "Name of gift card recipient?" and enter the recipients name in the box. *The name does not automatically input into the gift card. Purchaser can add in the "To" and "From" after they print the template. Recipient and purchaser names are tracked at the clinic for ease of use (in case of misplacement or has been forgotten at home). We offer physical gift cards for purchase at the clinic only.
