The manager of the Privacy Policy for Bliss Wellness is Roxanne Collins.

Respecting your privacy is important to us; we are committed to safeguarding your privacy. We have posted this privacy policy to help you understand how your personal information will be treated when you use our services. This policy may change from time to time, so please check back periodically.

Collection of information 

We collect only information that is required to provide safe, and effective health care. We request that patients provide us with general and emergency contact so that we may notify you of any changes to time or cancellation of appointments. We also record your address so that we may provide information for the purpose of invoicing. Files are kept in a secure location for 10 years after your last appointment, as required by RHPA. Patient files are the property of Bliss Wellness. If your therapist leaves the clinic, your file will remain the property of the clinic. Once your personal information is no longer required, it will be destroyed. All information is kept electronically. 

Access to files and information

All practitioners at bliss Wellness have access to your health records; however, only those providing you with treatment will view and update your file. If you have consented to a referral to another healthcare practitioner, then basic information will be forwarded to that practitioner. If information is required by any third party, such as an insurance company, lawyer or other health care provider, your written consent will be required before any information or documents are released. If you wish to have us consult with a specific health provider, we will ask for your verbal consent.

Patient access to files 

Patients have the right to see and read their files. A written request must be received 7 business days in advance. The privacy manager will assist you in understanding the information in the file. Patients have a right to request changes to the personal information in their file. In the event that the privacy manager and the patient cannot agree on an amendment to the information, such disagreements may be taken to the Information and Privacy Commissioner who may review the situation. We will request reasonable payment for any photocopies, consultation or summary of patient files. We will provide you with an estimate of the total cost in advance of processing your request.  

Communication

We will use the patients preferred method of communication. This may include phone, voicemail, text messages or email.

Information Automatically Collected

Information collected via this website is limited to what we need, in order to provide you with the best possible service. It will not be sold, or rented to any individual, group, corporation or agency. Release of your information occurs only when we have your consent; or when it is required to deliver a requested service to you; or when we are required or permitted to do so by law.

Like other websites, this one automatically gathers anonymous information about you such as your Internet address, the date and the time you visited the site, and the pages you viewed. This information is collected specifically to better understand how the website is being used, and to improve its effectiveness.

Email Address and Contact Information

When you send us an email, or when you request an appointment with us, we acquire your email address. We use your email address to acknowledge your appointment request and/or reply to your questions.We may also use contact information collected on this website to contact and correspond with you regarding the services that you requested and other related services we may offer.

 

Booking Services

When you book appointments through our website, we collect and store information about you necessary to complete your booking and to ultimately provide you with the requested services.

 

Use of Cookies

As part of our ongoing effort to personalize our services and to make your experience with us an enjoyable one, we may use cookies to store and retrieve information about you, and your usage of our website.
